Completed forms can be placed in an envelope with proper fees and mailed to: St. Mary's Religious Education, 211 N. Franklin Street, Polo, Illinois 61064, or place the envelope with the form and fees in the church collection basket no later than Sunday, August 30, 2009. Checks can be made payable to St. Mary's.
